Important Forms

​Use this form to be reimbursed for purchases made for the school or PTA events for which you would like the PTA to reimburse you.  These items must be previously approved by the PTA and included in the approved school year budget.

Fill out reimbursement form, attach invoice or receipt and submit to the PTA mailbox for approval. Reimbursements 30-60 days for payment to be received.

PTA members are use this form when taking in money for events or fundraisers.  At the end of the event the cash box is to be counted by two people and each is to sign the cash verification form. The cash is then given to the designated person (chair, president or treasurer) and the cash is deposited  and form is kept in the financial records.

Membership Form

All parents and staff wishing to become a PTA member can fill out this form and send it to the main off to be put in the PTA mailbox. Price is $15 per member.
